CONTRACT DURATION: 18 Months

To help fulfill its mission, LOFT is hiring a Coordinator.

Arleta and Shoreham Supportive Housing provides specialized assisted living and case management support and Life Enrichment Services to vulnerable older adults 59+ with complex physical health challenges, addiction and/or cognitive impairment living in two locations in the North West part of Toronto. The program offers 24/7 personal support services care through its specialized PSW team. Our Case managers provide care coordination, crisis intervention, system navigation and client advocacy to support individuals to live successfully in the community. Our Life Enrichment Services provide an opportunity for people to participate in meaningful programs that promote health, wellness and community connection.

Job Summary

The Coordinator provides team leadership in the overall service delivery and day-to-day operations of the Service. They provide a range of assistance and support in facilitating optimal client care, to support older adults and seniors living within the supportive housing buildings where LOFT provides services.  The Coordinator acts as the main link between clients and hospitals and community partner teams and facilitates assessment and intake process. The Coordinator provides clinical leadership and supervision around the Personal Support Worker scope of practice to ensure quality care that is aligned with LOFT Mission and values.  A key component of the position will be maintaining and building a support network of community resources to empower clients to live with dignity in their communities.

What You Will Do
In conjunction with the Program Manager, the position facilitates the operation by:

  • Overseeing all aspects of the site management of the Service.  This includes managing all health and safety requirements and general maintenance and security;
  • Promoting the personal empowerment of each client by assisting in creating a program and programming that builds a rapport through coaching, mentoring and support, enabling each to meet the daily requirements of their living environment; advocating on their behalf in navigating the social services and health care systems to ensure accessibility and availability of services; mediating issues or conflicts between clients, or tenants and neighbors, teaching conflict resolution; following-up on issues with clients to ensure resolution;
  • Providing direction and support to staff involved in direct service delivery; scheduling staff, assigning duties, assisting in the selection, orientation, training and performance management of staff;
  • Assisting with recruiting and hiring; and ensure LOFT policies and procedures are operationalized through developing and implementing operating procedures for the site and ensuring compliance with established standards;
  • Maintaining a general awareness of the daily activities and client issues and identifying staffing issues;
  • Develop cooperative action, build staff relationships and act as a liaison between staff and residents;
  • Liaise with housing provider to ensure that the health and safety standards are in compliance and support the petty cash;
  • Participating in the regular service planning processes which includes preparing the service plan, operating plan, client satisfaction feedback process, quality improvement and other research activities; identifying gaps in service and recommending solutions;
  • participating in meetings, planning and evaluation initiatives, committees, events and activities; compiling program evaluation reports and statistical information, inputting as required;
  • Organizing and participating in client meetings or other group activities which promote the wellbeing of the residents and the smooth operation of the service; providing other related assistance as required;

What You Bring
Must Haves

  • A degree/diploma in health care/social services or other related field with a minimum of five years relevant experience in the mental health sector
  • The position requires an excellent understanding of harm reduction and psychosocial rehabilitation principles as acquired through a post-secondary degree/diploma in human services, coupled with at least five years of experience providing community support services to the client population in order to provide team leadership in the application of the theories to practical settings.
  • Knowledge is required of the issues faced by older adults and seniors with special needs and of a marginalized client group which can include addictions, cognitive impairments, and complex health issues.
  • Experience in program development, planning and evaluation.
  • An understanding is also required of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.
  • Experience in conducting meetings or facilitating groups, good analytical, time management, and organizational skills to co-ordinate and follow-up on issues or activities with clients.
  • Excellent interpersonal, communication and counselling skills are required to provide team leadership and support and to engage the clients in some highly personal areas.
  • Advocacy skills with knowledge of local community agencies and other professional services available for the client group and an ability to establish effective relationships with service providers, clients families and friends.
  • Good intake assessment, crisis intervention, advocacy and problem solving skills.
  • Computer literacy sufficient to produce and maintain files, records or statistical information.
  • A valid Ontario driver’s license may be required for the position
  • Availability to be on after-hours on call, on a rotating schedule
