POSITION: Nurse Practitioner (Indigenous Health)
LOCATION: 22 Vaughan Road, Auduzhe Mino Nesewinong Indigenous Health & UHN Site
SALARY RANGE: $75/hour (NP) variable shifts (9:00am – 5:00pm, occasional weekend, subject to
change)
START DATE: Immediately – *Two-year contract (Full benefits after 6 months probation)
Background: UHN’s Social Medicine Program (SMP), and Auduzhe Mino Nesewinong is hiring a Nurse
Practitioner (Indigenous Health) to address the social determinants of health for patients with unmet
complex medical and social needs. The SMP provides care for patients that frequently require care services
at UHN’s Emergency Departments and other units. The Nurse Practitioner (NP) (Indigenous Health)
position is intended to partner and support the SMP interprofessional team with providing Indigenousspecific health services to create a supportive and culturally safe space for Indigenous patients in various
hospital settings, the Stabilization & Connection Centre, Winter Discharge Centre, UHN Social Medicine
Housing, and mobile health services in the community. The NP will partner with Auduzhe Mino
Nesewinong (Place of Healthy Breathing), an evolving Indigenous Interprofessional Primary Care Team
located in Tkaronto. Auduzhe has been advancing to include vaccination, preventative, episodic and
primary care to aim to meet the voiced needs of the First Nations, Inuit and Metis community. Auduzhe
focuses on disease prevention, vaccines across the lifespan, health promotion, wholistic care, and data
sovereignty, through outreach partnerships, home visits, and on-site healing. The NP (Indigenous Health)
will partner with patients to support them in addressing their complex needs, including providing primary
care support, navigation to community-based health and social services, and referral to specialists, mental
health, and harm reduction support.
Position Summary: One day per week, the NP (Indigenous Health) will provide culturally safe and
comprehensive care for individuals seeking support from UHN’s SMP with an emphasis on primary care
and the social determinants of health. The remaining 4 days they will work onsite at Auduzhe Mino
Nesewinong to provide care for individuals and families seeking support. The NP (Indigenous Health) will
use advanced Nursing knowledge, skill, and judgment to assess and manage clients who may present with
complex social and health conditions such as unmanaged chronic disease, mental health, substance use,
sexual health, and the ongoing effects of colonialism. The NP (Indigenous Health) works collaboratively
with the interprofessional team to provide outreach and on-site episodic, transitional, primary care, and
public health services to clients respecting PHIPA and Indigenous privacy. The Nurse Practitioner
(Indigenous Health) will be expected to perform cross-functional and/or other duties consistent with the
job classification, as assigned or requested.
Roles and Responsibilities:
▪ Provide cultural and safe care to First Nations, Inuit, and Metis community members in Tkaronto.
▪ Assess and address any unmet acute/ primary clinical and social needs that arise during operating
hours
▪ Conduct comprehensive health assessments, provide testing, management, counselling, and
prevention services
▪ Order and interpret results of screening, diagnostic lab tests, x-rays, ultrasounds, lab/ bloodwork
and other diagnostic screening
▪ Participate and provide support to programs and community services which are in the interest of
public and holistic Indigenous health
▪ Collaborate with other Indigenous and service provider organizations to develop and implement
culturally safe defined referral pathways
▪ Maintain accurate and timely electronic clinical documentation (consistent with the College of
Nurses of Ontario Nursing documentation standards).
▪ Work in collaboration with interprofessional team members and Indigenous and allied service
organizations (as required) to ensure continuity of care and safe transition(s) between care sites to
ensure patient’s needs are met
▪ Maintain and develop ongoing competency through appropriate continuing education
Qualifications:
▪ Successful completion of Master’s Degree Program in Nursing
▪ Successful completion of a College of Nurses of Ontario recognized NP Program
▪ Current registration as a Registered Nurse in the Extended Class and in good standing with College
of Nurses of Ontario
▪ Four to six years of practical and related nursing experience
▪ Two years of experience working as an NP in primary care, mental health and addictions preferred
▪ Demonstrated knowledge and experience engaging in, and working with Indigenous/ socially
vulnerable communities
▪ Ability to contribute to culturally safe and inclusive Indigenous care
▪ Proficiency providing trauma-informed care using a harm-reduction approach
▪ Ability to work independently in unique community-based primary care settings
▪ Cross-appointed (or eligible) to an accredited University
▪ Membership in a professional organization (ie. Registered Nurses Association of Ontario, and/ or
Nurse Practitioner Association of Ontario), and/ or other organizations relevant to the role
▪ Excellent interpersonal and communication skills
▪ Excellent organization and time management skills
▪ Excellent clinical reasoning, decision-making, problem recognition and problem-solving skills
▪ Knowledge and ability to efficiently navigate and use electronic medical records
▪ Current CPR/ ACLS certification
▪ Evidence of Professional Liability Protection
