To help fulfill its mission, LOFT is hiring a Case Manager.

Job Summary

The Case Manager provides a range of services that supports individuals living with serious mental illness, addictions and physical health issues. The Case Manager’s goal is to empower clients, thereby increasing their independence, dignity and self-respect in a safe and secure environment. This position liaises with other community agencies and professional services.

The Crosslinks Street Outreach and Services Network Program provides a mobile van service that operates seven days a week from 2 pm – 9 pm, except for statutory holidays in York Region. Using a harm reduction approach, the program provides services to residents of York Region experiencing homelessness and those living at low and moderate-income levels.  Clients are able to receive support from the van for basic needs and life-saving services in a manner that fosters self-worth and dignity.

What You Will Do:

  • Develop a plan of support and recovery, which includes identifying priorities and goals, in conjunction with client, other professionals, areas identified through initial intake and OCAN’s.
  • Promote the personal empowerment of each individual through coaching, mentoring and support.
  • Directly assist with their daily living activities, particularly in the acquisition of life skills such as medication management, cooking, cleaning, shopping and budgeting.
  • Establish and co-ordinate a support network of social, family, medical, psychiatric and other community links.
  • Assist clients with getting connected to housing
  • Provide crisis intervention/management and assist in the development of crisis plans with clients.
  • Support program administration by completing and maintaining the required records, assessments or statistics such as client files.
  • Do outreach at various drop-in programs
  • Do outreach shifts a minimum of 3 times per month (1-9 pm shifts)
  • Contribute to program development and implementation
  • Attend training sessions and team meetings
  • Have up to date knowledge of York Region resources in order to facilitate appropriate referrals.

What You Bring:
Must Haves

  • Completion of a degree/diploma in Social Work/Human Services or equivalent.
  • Minimum of three years’ experience in providing support to vulnerable adults living with serious mental health issues and/or concurrent disorders and/or justice involvement and/or complex needs.
  • Ability to directly assist clients with daily living activities, promoting the acquisition of life skills.
  • Understanding of psychosocial rehabilitation and recovery principles, harm reduction, and the therapeutic relationship and the ability to apply these concepts into practice.
  • Knowledge of issues faced by adults with serious mental illness and addictions and of available community and other professional services in York Region/funding is specific to York Region
  • Creativity and flexibility to approaches.
  • Good oral and written communication skills, counselling and responding skills to engage clients in highly personal areas.
  • Ability to work effectively within a team environment and to interact with workers from a variety of agencies.
  • Ability to implement assessment tools, develop and implement care plans in conjunction with client.
  • Crisis intervention skills, with well-developed problem solving abilities.
  • Possession of a valid driver’s license use of a reliable vehicle is required and insurance
